Paediatric outreach team
The Paediatric Outreach Team provides a range of services to support children aged 0-16 years old (0-19 years old with complex medical needs) at home and in other community settings including schools The Team care for children at home to reduce hospital admission, minimise the length of stay and promote early discharge whenever it is safe to do so. We work in partnership with children and their families/carers with both short and/or long-term nursing needs and also offer an in-reach service for short term treatments.
The service is provided by an experienced team of children’s trained nurses. We work alongside paediatric teams at Stoke Mandeville and High Wycombe Hospital, specialist hospitals such as the John Radcliffe Hospital (Oxford), Great Ormond Street Hospital (London) and Kings College Hospital (London).
We act as a link between the hospital and the community helping to coordinate services, promote continuity of care, promote care at home and ensure a seamless transition home from hospital. We provide specialist nursing advice and care for the following:
- Administration of medications – including intravenous, subcutaneous and intramuscular medication
- Central line care – chemotherapy at home, blood sampling and dressing changes
- Enteral Feeding – management of children with gastrostomy devices and the teaching and management of nasogastric tubes
- Respiratory Conditions – including oxygen care at home, tracheostomy care and ventilator dependent children
- Wound care – post operative, trauma and pressure areas
- Renal – HSP, nephrotic syndrome and glomerulonephritis follow up
- Urology – home dialysis, catheterisation, and supportive care
- Palliative care – end of life care with a 24 hour on call service when required
Any nursing procedure that can be safely undertaken at home or in a community setting
The geographical area we cover:
The team cover the majority of Buckinghamshire. If your child is cared for by a Buckinghamshire Healthcare Consultant, visits can be arranged crossing the Buckinghamshire border into nearby counties.
Referrals:
A child must be under the care of a Buckinghamshire Healthcare NHS Trust Consultant for our team to visit them. Referrals are accepted from all healthcare professionals for any child with a nursing need and can be made by your hospital consultant, school nurse, health visitor or GP.
Working hours:
We offer a 7 day a week service
Monday – Friday 8am – 6pm
Weekends and Bank holidays 10am – 3pm
