Central Digital Wound Hub for remote post-operative wound monitoring

What is the Central Digital Wound Hub (CDWH)?

It supports patients following specific types of surgery across Buckinghamshire Healthcare NHS Trust. It’s a remote wound monitoring service that enables your surgical team to monitor your wound healing and provide advice based on the information and images you share.

This service helps:

  • reduce the risk of wound infection
  • support earlier detection of complications
  • improve healing outcomes through timely review and advice

Please note this is not an emergency service.

What can I expect?

You’ll get secure weekly messages from us (via SMS or email) asking you to:

  • submit photos of your wound
  • provide short updates on how your wound’s healing.

These requests will come from Buckinghamshire Healthcare NHS Trust. Isla Health provides a secure system to help manage the requests.

You’ll need to:

  • provide your year of birth
  • give consent to participate (you can withdraw at any time by emailing us).

What happens next?

A trained team of healthcare professionals will review your photos and information. You’ll usually get a response within 24 hours, often much sooner.

The CDWH team may contact you for more information or follow-up questions.

We monitor most patients weekly for 37 days, or until their wound has healed and they are discharged from the service.

For hip and knee patients we may check in monthly for up to one year.

If we have any concerns, we may:

  • recommend you visit A&E, an urgent care centre, or the Trauma Assessment Unit at Stoke Mandeville Hospital
  • organise a return to hospital if needed
  • coordinate care with your GP, practice nurse, or district nursing team where we think community-based support is more appropriate.

Is it free to send photos?

Yes. There’s no charge to send wound photos or updates through the system.

Is my information secure?

Yes. Isla Health is a secure, NHS-approved platform. It uses encryption, access controls, and audit logs. It’s GDPR compliant.

Do I need to send updates if my wound is healing well?

Yes. Please send regular updates even if everything looks fine. This helps your team monitor progress and catch issues early.
