Adult community healthcare teams (ACHT)
The adult community healthcare teams provide support to patients in their own homes, by providing 24/7 nursing care and assessment. They also provide physiotherapy and occupational therapy assessment and the provision of short-term equipment.
ACHT deliver an enablement service which provides short-term care and rehabilitation to patients in their own homes to facilitate discharge from the acute sector or to prevent acute hospital admission.
Thame Day Hospital
This runs two days per week (Monday and Thursday) and provides a full multi-disciplinary assessment and rehabilitation programme to inpatients and referred clients. A falls prevention programme, pulmonary rehabilitation and a programme for patients with chronic respiratory problems are also provided.
Outpatient services
There are approximately 22 separate consultant, nurse and therapy-led specialist clinics run at Thame Community Hospital to enable patients to access these services more locally.

Musculoskeletal physiotherapy outpatients services
The musculoskeletal physiotherapy team provide an outpatient service Monday – Thursday, including some evenings. Access to this service is via GP referral. Physiotherapists are experts in rehabilitation, helping people return to their normal activities after illness and injury, and can advise on building easy, effective exercise into your daily routine.
Health visitors
Health visitors provide a family focussed service and work in partnership with other agencies, specifically health, education, social care and voluntary organisations, to support the well-being of all users of the service. The team can support the whole family and can advise, listen and direct families to find specialised help for coping with problems. The health visitors service runs Monday – Friday.
Podiatry services
The podiatry team operates an outpatient service Monday – Friday. Access to this service is via GP or self referral. The podiatry service deals with the assessment, diagnosis and treatment of foot and lower limb conditions. Following assessment, patients requiring podiatry intervention will be offered a treatment plan, with the aim of improving mobility, reducing pain and increasing independence. Thame and District Day Centre
Thame and District Day Centre is funded by Oxfordshire Social Services and meet at Thame Day Hospital on a Tuesday, Wednesday and Friday each week from 9.30am – 3pm. The day centre opens for 50 weeks of the year and provides a day out with a hot meal, entertainment and refreshment for the local community who are elderly, frail or, possibly, isolated.
