Head and Brain Injuries

Our Community Head Injury Service (CHIS) supports people with acquired head and brain injuries.

We help you, your family or anyone who experiences difficulties following a head injury or an acquired brain injury.

Who we see

We assess, rehabilitate and provide family services to people aged 16 and above with a brain injury. This may include:

  • traumatic brain injuries (head injuries)
  • cerebral hypoxia
  • encephalitis
  • meningitis
  • subarachnoid haemorrhage
  • some other forms of stroke (in specific circumstances)
  • cerebral tumours.

How to access our service

If you are registered with a GP in Buckinghamshire your GP or other health professional can refer you to our service via our email address:

buc-tr.CHIS@nhs.net

You can also self refer, or a member of your family can refer you at any stage after your injury. This could be after you have been discharged from hospital, during the early months and years of recovery, or if you’re struggling to adjust to the long-term effects of a brain injury.

Our team

Our interdisciplinary brain injury team includes clinical neuropsychologists, occupational therapists, a brain injury practitioner, a physiotherapist, a physiotherapist, a speech and language therapist, a work placement consultant and rehabilitation assistants. We also have access to rehabilitation medicine specialist clinics outside of our team.

Our services

We provide specialist person-centred, goal-directed community brain injury rehabilitation, facilitated by a key worker, across 3 linked services – Rehabilitation Team, Working Out Team (specialist Vocational Rehabilitation) and Family Services.

We deliver our services at the Brookside Clinic in Aylesbury (see map and image of building below), as well as at your home, and in community locations including the workplace, depending on your individual needs.

Our core operating times are 9am to 5pm, Monday to Friday.

These may include a combination of individual therapy (clinical neuropsychology, occupational therapy, physiotherapy, speech and language therapy) and specialist brain injury group programmes, for example: brain injury education, cognitive skills, communication skills, fatigue management as well as technology, cooking and woodwork groups.

‘Working Out’, helps adults return to work and retain their jobs after all forms of acquired brain injury (including stroke, subarachnoid haemorrhage and cerebral tumours).

The programme provides specialist brain injury vocational assessment, work preparation, trials, supported placements and job retention interventions. This may involve a combination of individual work and specialist group programmes, for example a work preparation group, community vocational rehabilitation activities, job club and a placement support group

Family services include carer counselling, relatives’ brain injury educational programme, follow-up workshops and couple/family counselling.
