Who is the service for?
Patients who require monitoring of their Warfarin or Sinthrome treatment are referred by their GP or by hospital consultant.
What does the service provide?
The service provides clinics for patients to attend to have point of care blood tests-finger prick for INR (International Normalised Ratio) this determines the dose of anticoagulant drug required between blood tests.
Who will the patient meet?
Patients will meet specialist trained anti-coagulation nurses or pharmacists in an outpatient or community clinic setting.

The service also provides advice and education to nursing and medical staff in relation to pressure ulcer prevention. Secondly to provide an expert advisory service to the nursing and medical staff for all aspects of woundcare.
The types of wounds reviewed by tissue viability include pressure ulcers, leg ulcers, diabetic foot ulcers, surgical wound breakdown
