Rapid Response & Intermediate Care

Our innovative Rapid Response & Intermediate Care (RRIC) service provides urgent care for patients in crisis in the community allowing them to remain safely in their own home and empowering recovery.

The Rapid Response and Intermediate Care service (RRIC) is available for patients who are registered with a Buckinghamshire GP. The RRIC team provides the following:

  • an urgent response and intermediate care if you experience a health crisis at home to help avoid the need to go to hospital
  • swift health interventions to allow you to return home from the emergency department so you don’t need to be admitted to hospital
  • intermediate care at home to allow you to leave hospital
  • community physiotherapy
  • support to enable dignified end of life care
  • health interventions and/or rehabilitation based on personalised goals to help you recover from an episode of ill-health, maximise your independence and the quality of life in your own home

Patients are referred to our Rapid Response & Intermediate Care (RRIC) service by health and social care professionals. (Please note it is not possible to self-refer).
