School nursing
We help your child to make the transition from early years settings to primary school. We support children and young people in Buckinghamshire schools and the local community with medical, social, emotional or physical needs.
Our team helps each person achieve their potential. They’re also involved with vaccinations and preparing your child for school.
Our support continues into secondary school and in some cases, college life and making the transition to adult services.
Who we are
We’re a team of multi-skilled staff who specialise in child health and wellbeing. This includes specialist community public health nurses, community nurses, child health and wellbeing advisors, and school health assistants.
We work with colleagues in our NHS Trust’s safeguarding children’s team and our health visiting team who support pre-school children.

We work in 3 teams across Buckinghamshire:
- North Bucks (including Aylesbury and Buckingham)
- South Bucks (includes Chiltern and South Bucks)
- Wycombe

How to access our services
Complete our school nurses e-referral form
You must include detailed information in each section of the referral.
Who can make the referral?
A parent, carer or healthcare professional.
Conditions for the referral
The child or young person:
- must be over aged 4 years 6 months and attending a mainstream school in Buckinghamshire
- can’t have similar care and support for the same reason from another service.
The referral must demonstrate:
- consistently implemented self help strategies with little positive sustained change.
- that the parent/carer has completed a relevant parenting programme and implemented the strategies with little positive sustained change.
You can’t refer a child or young person to another specialist service for an assessment at the same time.
If a child/young person has a social worker
We need a copy of the latest plan. If this isn’t available, it will delay the assessment and triage process.
Send the plan securely to schoolhealth-referrals@nhs.net We’ll add it to the child’s records for review.
A GP or paediatrician will need to refer children aged 7 or older to our bedwetting clinic.
