Changes to home-based hospice care provision in Buckinghamshire

From 1 January 2025 Buckinghamshire Healthcare NHS Trust Hospice@Home and palliative care community services will align more closely with specific groups of GP practices to provide a more integrated service across the county.

Healthcare commissioners have awarded new contracts for Hospice@Home to the main providers of hospice care in Buckinghamshire to encourage closer coordination between hospice care providers, district nurses and GP surgeries to provide joined up care for those with an end-of-life diagnosis in each local area.

From January 2025 Florence Nightingale Hospice will provide Hospice@Home care in the following areas:

Matron for Palliative and End of Life Care services at the Trust, Liz Monaghan said:

“The Florence Nightingale Hospice team has provided end of life nursing care for people both in the hospice at Stoke Mandeville Hospital and in their own homes for over 30 years. We look forward to working more closely with GP colleagues and to extending the care we provide through the Hospice@Home service to a greater number of patients and their families in over half of Buckinghamshire.”

The Florence Nightingale Hospice situated on the Stoke Mandeville Hospital site will continue to offer palliative and end of life care to people from the whole of Buckinghamshire. This includes inpatient care, day hospice and Lymphoedema. The Florence Nightingale Hospice team also provide palliative and end of life care for inpatients at Stoke Mandeville, Wycombe and Amersham Hospitals.

Home-based hospice care in the rest of Buckinghamshire

Hospice@Home care in other areas of the county will be provided by the other two main Hospice services in Bucks Rennie Grove Peace and Thames Hospice. The areas that will be covered by these providers are:
