Our Community Neurorehabilitation Service sees adults with a wide range of long-term neurological conditions, such as multiple sclerosis, Parkinson’s or people who have had a stroke. These conditions often result in a complex combination of difficulties (for example physical, emotional, communication and cognitive), requiring input from a variety of disciplines.
Our aim is to assist people in retaining their independence and in maintaining their personal, family, work, leisure and social activities, whilst managing their condition.
The service is based at Rayners Hedge in Aylesbury and Amersham Hospital. Physiotherapy is also currently offered at Chalfonts and Gerrards Cross Hospital.

The inter-disciplinary team has access to clinical neuropsychology, occupational therapy, physiotherapy, rehabilitation medicine, and speech and language therapy. The team is supported by a therapy assistant and administrative support. We work closely with other health and social care professionals, and voluntary organisations, as required.
The service provides specialist person-centred, goal-directed neurorehabilitation programmes facilitated by a key worker. Our service is provided in a flexible manner according to individual need, via centre-based appointments, either individually or within a group. If required, rehabilitation can be offered in the individual’s home. According to the needs of the individual, rehabilitation may be offered by a single discipline or by the inter-disciplinary team.
Family members are included in the rehabilitation process, as appropriate. We regularly run groups such as Fatigue Management, Support and Advice for Stroke Survivors and the Parkinson’s Group.
The service is available 9am-5pm, Monday to Friday. Where there is a specific need to accommodate individual circumstances, earlier or later appointments can be offered.

The multi-disciplinary team has access to occupational therapy, physiotherapy, clinical neuropsychology, rehabilitation medicine, speech and language therapy and administrative support.

Our service is provided in a flexible manner according to individual need, via centre-based appointments, or rehabilitation in the individual’s home. According to the needs of the individual, rehabilitation may be offered by a single discipline or with input from other health professionals. Family members are included in the rehabilitation process, as appropriate.
Every effort will be made to offer an appointment at a mutually convenient time. However, we are currently only able to offer appointments during office hours Monday to Friday.
A small ‘satellite’ physiotherapy service operates at Chalfont’s and Gerrards Cross Hospital on Tuesdays and Thursdays.
