Buckinghamshire Falls Prevention embraces a model which ensures that falls prevention is everyone’s business.
It is a whole systems approach maximising the use of the knowledge and skills of staff in many services.
We aim to ensure that the falls prevention pathway in Buckinghamshire enables people to access the most appropriate level of assessment and treatment. Geriatrician level assessments for complex and unexplained falls; therapy or nursing led multifactorial falls assessments for those who need them, brief falls assessments for those who do not need the expertise of falls specialists.
In order to support the pathway a Falls Risk Screen is in place. The Falls Risk Screen must be completed by a health or social care professional before referring an older adult for falls prevention interventions.
