Rehabilitation aims to maximise neurological recovery, functional abilities and psychological adjustment, so you can return to the community as independent and productive as possible - and prepared to resume your life.
You are at the centre of our multidisciplinary team, this helps to ensure we reach your full potential when goal planning. You will actively participate in this process and your achievements will be acknowledged and your next goals set, by you and your treating team, at the three-weekly goal planning meetings.
Our ultimate outcome is for you to be safely discharged from the centre and reintegrated back into your family and community in a way that meets your own wishes and needs.
Our pre discharge rehabilitation ward (St Joseph) is designed to promote your own independence – where you have the support and security provided by our experienced nursing staff. You will be transferred to this ward before you leave hospital at a time that has been agreed through your goal planning and based on your needs.
Before being transferred you will receive a welcome letter of introduction and on arrival informed of your named nurse. You will have a full timetable of rehabilitation activities and you will be encouraged to try different equipment.
When on the ward, breakfast is provided in the kitchen and other meals in the dining room, unless it has been agreed you can cook and cater for yourself or other circumstances prevent this (for example, due to infection control).
Relatives and friends are welcome on the ward - visiting hours are 9am to 11pm. Should you require any input from our staff, relatives will be asked to leave the area unless you have given permission for them to remain.
