Until recently (March 2010) community health services were provided by Buckinghamshire Primary Care Trust. We are positive our new integrated community and acute care Trust will bring fresh opportunity to improve links between healthcare professionals, create more seamless patient care pathways and deliver more services closer to home.

Waterside and Chalfont Community Hospital

Following the integration of the Trust with Community Health Buckinghamshire, we are now in a great position to better utilise our community facilities and ensure our patients are seen in the most suitable environment for their care. As a result, we are expanding our existing service provided in Waterside and Chalfont Community Hospital at Amersham Hospital (Chartridge ward).

The Waterside and Chalfont Community Hospital is a nursing and therapy led unit with medical support from GPs. The service supports re-enablement and sub-acute medicine for older people and transitional care for patients awaiting placement in a nursing or residential home.

This is a temporary measure while we continue to develop and grow our integrated care teams to provide care for our patients closer to home.
