How do I make an appointment?

Where possible, please give us two to four weeks notice so the appointment time can be used for another patient.

What should I bring to my appointment?
Please bring with you a list of medication and your glucose diary and\or glucose meter. Remember to have your blood tests one to two weeks before the appointment, if requested.

What time should I arrive for my appointment?
Please arrive 15 minutes before your appointment to allow time to book in and for the nurse to measure your height, weight and blood pressure.
