Community Assessment and Treatment Service (CATS)

We assess and treat frail and older patients with complex needs to reduce their likelihood of going into hospital.

We identify illnesses that need treatment and things that may help our patients to feel safe and independent. Our physiotherapist and occupational therapist can assess and provide mobility aids, and equipment at home such as grab rails.

Some patients come to us because they need medication reviews, catheter care or IV therapy.

What to expect

After we assess you, our team will offer help and advice and discuss any changes that we make to your treatment.

We’ll develop a care management plan for you and send this directly to your GP on the same day that we discharge you.
